Each Point in Use

For nine articles you have been learning where the points are and how they are given. This article turns the atlas over and reads the other side of every card: what each point is for, and who must not be given it. It is the article a therapist opens with a patient lying in front of him and a half-written card in his hand.

How to read this article

Read the right column first; the bracket is a selector, not a dose

The atlas taught you a verb. This article teaches you a sentence.

The Front of the Body I to 24 answered how: where the patient lies, where the hand or the foot goes, how many times, at what angle, in what order. Not once did they answer why this patient. That was deliberate. A student who learns the indication before the technique starts fishing for diseases; a student who learns the technique first has something to hang the indication on.

Now the hanging is done, and it has two halves.

Three habits make this article usable rather than merely long.

First — the number in the bracket is a selector, not a dose. This is the single most important sentence in the article and it is stated at the head of three separate families. Stimulating in different numbers produces different effects. (2) Adr raises sodium and is refused to a patient with high blood pressure. (4) Adr raises blood sugar and is refused to a diabetic. (6) Adr reduces inflammation and is refused to anybody whose immunity is already low. Those are not three strengths of one treatment. They are three treatments, and a prohibition attached to one of them does not travel to the others.

Second — read the right-hand column before the middle one. A therapist who reads the indications first will always find a reason to give a point; the list of uses is long and generous and some of it will fit almost any patient. Read the prohibitions first and you are asking the only question that can harm somebody: is there a reason this person must not have it? The story at the head of this article is that habit, arriving one week late. (Figure 25.2)

Third — a prohibition that names a substitute must be obeyed as written, substitute and all. Only a few entries do this, and they are the most useful lines in the article. Where Mu is forbidden, (4) Down arrow may be given in its place. Where (2) Pan is forbidden in a child being treated for height, (4) Pan or (10) Pan may be given. In heart patients, (2) Thrd is given in place of (4) Thrd. Where Gas Only must be given to a constipated patient, Gas 'I' must be given with it — and if the constipation is severe, (2) S4, S5 as well. A closed door with another door beside it is a rule a therapist can actually keep.

The three kinds of caution, and how each is to be treated

The caution reasoned from what the point does.

This point raises X; therefore withhold it where more X is harmful. (4) Adr raises blood sugar, so not in diabetes. Gas Only raises stomach acid, so not in ulcer. Gas 'I' increases the movement of the small intestine, so not in loose motions. Lu + Sh raises blood pressure, so not in high blood pressure or heart disease. Follow these, and learn the mechanism with them — the reason is the memory hook.

The caution learnt by watching. I saw this go wrong, and I do not know why. Gal in asthma increases the trouble — so it is not given. Gal is not given to those passing little urine, and the line attached to it says plainly: this is a matter of experience. Pan given to a patient with schizophrenia causes them to hear voices in the ears, so it is not given to them. These are the rarest and the most valuable kind, because nobody deduced them; somebody paid for them. Follow them exactly, and keep the same kind of record yourself.

The caution that protects a body still being built. (2) Pan stops growth hormone, so it is not given to a child whose height is less or who is under treatment to increase height. Lt. Ov. and Rt. Ov. are refused to the same child, because oestrogen and progesterone hasten the closure of the epiphyseal line of the long bones and the growth in height will stop before its time. These two are absolute and they are worth saying out loud to a parent, because a parent who understands them will not go looking for the treatment elsewhere.

Thirty-two codes, in nine families. Each table gives the point and its variant, what it is used for, when it must not be used, and the notes attached to it — including the article where its method is taught, so that the reference and the atlas stay bound together. (Figure 25.1)

The two adrenal points — cortex and medulla

Adr is aimed at the rind, Swt at the core — and the counts differ

The adrenal gland is two glands in one wrapper. The outer rind makes the slow chemicals — the salt-holding one, the sugar-raising one, the one that puts out inflammation. The inner core makes the fast ones, the two that a fright releases. ADR is aimed at the rind; SWT is aimed at the core. That is the whole of why they behave so differently, and why the counts under each open onto such different rooms.

Poisoning, envenomation and appendicitis are named in that list and each of them is a race. A (6) Adr given while somebody carries a child to a hospital is one thing; a (6) Adr given instead of carrying the child is another.

Read those last two rows across each other and you will see the finest piece of bookkeeping in the whole atlas. One point, split by side, becomes two opposite treatments. The left opens and the right closes. The left is refused to a diabetic; the right is refused to a hypertensive. Nothing else in this therapy is built quite so symmetrically, and a student who can recite the left–right split of Swt has understood what is meant by the count and the side are the treatment.

The organs of digestion

The largest family: every gland draws its raw material from digestion

This is the largest family in the therapy and the reason the belly is worked before anything else. Every gland in the body draws its raw material from what digestion delivers.

The pancreas — PAN

By Pan we stimulate the pancreas. Stimulating in different numbers produces different effects — and here, more than anywhere, the counts open onto rooms that have nothing to do with one another.

Given to correct hole in the heart (ASD, VSD, PDA). In heart disease or obesity, to reduce triglycerides — chiefly where the diabetes or heart disease arises from hypothyroidism. To end dead skin from tissues, to open old clots, and to protect the skin from bacteria, acting as an antiseptic. To correct cracking of the skin of the hands or the feet. To increase the number of white cells (?). If the mother of a cerebral palsy child had high blood pressure during pregnancy, giving that child (10) Pan will benefit (?). In kidney failure — uraemia, nephrotic syndrome — HIV and goitre, those who do not benefit from (2) Pan benefit from (10) Pan (?). The stomach point and its companion — the clearest compensation rule in the therapy

Gas Only stops loose motions. That is one of the things it is for, and it is also the reason it becomes a problem in the wrong belly.

So the instruction does not simply forbid it to a constipated patient. It says: if Gas Only must be given for some reason to somebody with constipation, piles or fistula, then Gas 'I' must be given with it — because Gas 'I' increases the movement of the small intestine and pulls in the other direction. And if the constipation is severe, (2) S4, S5 may also be given with Gas 'I'.

Three points, running together, each answering the last one's shadow. Learn the shape of this instruction, because you will meet it again in the formulary: a therapy that knows what its own treatments cost writes the payment into the same line.

The liver and the spleen

Notice the word nowadays. It appears twice in the Spl box — nowadays it is also given in inflammation, nowadays it is not given in cancer — and it is doing something a therapist should respect. It marks a rule that was different before and was changed on evidence. A teaching that prints its own revisions rather than quietly rewriting them is telling you that the atlas is a living record, and that your own careful observations have somewhere to go.

The bowel points — MU, CONST, DYS

Dys — the most self-restrained point in the therapy

Read the three brakes again, because between them they are a small course in how to hold a powerful treatment.

The gate. Give this only if there is pain in the upper part to the right of the navel; otherwise not. Not if the patient has loose motions — the complaint is not enough. There must be a finding under your fingers.

The ceiling. It may be given for a day or two. A day or two is not a suggestion about how long it takes to work. It is the outer limit.

The reason. Its use must be undertaken only with great thought — because it works against the natural motion of the intestines. Loose motions are partly the body emptying something it wants out. A treatment that slows the bowel is holding that something in. The therapy says this about its own point, in its own voice, and asks you to think before you use it. ▲ Take care

And there is a reason that warning is not squeamishness. The gut carries its own nervous system — a complete network of nerve cells in the wall of the intestine, enough of them to run the whole organ on their own; cut every connection to the brain and an intestine still senses what is inside it, still decides, and still moves it along in orderly waves. Which means a bowel that has speeded up has usually decided to speed up, and something is in it that its own wiring wants gone. It works against the natural motion of the intestines is respect for a second brain that knows more about the situation than the therapist does.

The two kidneys and the two directions

Liv° reduces alkali, Mu° reduces acid — learn the two as a pair

Everything in this therapy that has a left and a right has a reason for it, and nowhere is the reason plainer than here. The left kidney point pulls the body's chemistry one way; the right kidney point pulls it the other. Learn them as a pair or you will learn neither. (Figure 25.3)

The direction the whole system agrees on

Liv° reduces alkali — and so corrects high blood pressure.

Mu° reduces acid — and so corrects low blood pressure.

(6) Adr reduces acid and raises alkali — therefore do not give in high blood pressure.

Pit raises alkali — so do not give to high blood pressure, cancer and fits patients.

All four agree, on pages that never mention one another: alkali is the direction that raises pressure; acid is the direction that lowers it. That agreement is what tells you the atlas was thought through rather than assembled. And it is why Acid is useful in low blood pressure and (1) Acid is refused to a patient with no pain at Mu° — the left-sided, acid-reducing kidney point and the acid point belong to the same account.

The chest and the breath

Two things in the Lu + Sh row deserve saying out loud. The first is that this is one of the treatments taught to the mothers — a point given daily at home, by a parent, to a child who drools. Much of what this therapy achieves in children it achieves because somebody was taught to give it on a kitchen floor between meals.

Read the two halves of that row together and the rule assembles itself.

Inside the 1,25 DCC formula, (4) Para maintains the calcium in the blood — the parathyroid hormone working with the other points of that formula.

Given separately, by itself, it does something else: it lifts calcium out of the bone and puts it into the blood. That is precisely why it is the point used where bone has grown at some unwanted place.

So for a patient whose bones are already thin, the bar is not never give (4) Para. It is never give (4) Para alone; give the whole formula. The prohibition is aimed at the word separately, and a therapist who reads only the point code and not the word beside it will get this wrong in exactly the patient who can least afford it. ▲ Take care

An empty right-hand column is not the same as a safe point. It means the therapy names no condition in which this point is harmful — and it names none because this point gives, rather than drives. But the standing rules still hold: a patient who cannot be placed on her side is not placed on her side; a hip that has been fractured or replaced is not loaded; and a numb leg cannot report what your foot is doing to it.

KU — the one point whose counts are seconds

Ku is unlike anything else in the chemical half. Everywhere else a bracketed number counts applications. Here the number is very often a duration, and the entry states its arithmetic before it states a single use. (Figure 25.5)

For all the rest of the Ku entries, give *equal* — that is, for as many seconds as are written, give it only once.

So (½) Ku – 20 secs is one application, held for twenty seconds. Not twenty presses, not twenty of anything. Read the word secs on the card and the whole meaning of the number changes.

Ku is the only place in the atlas where the therapy says, of its own treatment, that the work is unfinished — research on these is continuing. Keep the sentence. A student who knows which parts of a system are settled and which are still being explored can use both honestly; a student who is told everything with the same confidence cannot use either.

The chemical points end not with a summary but with a single sentence, set large across the foot of the page. (Figure 25.6)

Remember — whatever the disease may be, giving the appropriate chemical point to correct the LMNT point at which there is pain will of itself bring relief in the disease.

Read it as an instruction and it is the discipline that runs through every entry above: treat what you can find under your hands, not what you have been told the diagnosis is. That is why almost every row in the tables above begins with a pain — pain to the right of the navel, pain just to the left of the navel, pain at the point in front of the pelvic bone — and not with a disease name.

And the same pages that print it have already told you where it stops: the points withheld in cancer, in kidney failure, in pregnancy, in menstruation, from a growing child; the claims marked with their own question marks; the treatment refused the name of contraceptive until the research is done. Take the sentence as permission to stop asking what is wrong and you have contradicted eighteen pages of the same teaching.

The second half of the atlas is named differently and reads differently. These treatments are called after a place on the body, a movement of the hand, or the person in whom the treatment was first worked out — Back arrow, L5–S1 ghisai, Giriraj, Pradip, Shivaji, Sudhakar, Vasanti. Their entries do not talk about hormones. They talk about tissue that has come down and must be pulled back up, a tendon caught inside a band, a joint that has stiffened, a vertebra out of its place.

That changes what a prohibition means here. A chemical point is withheld because the body is already making too much of something. A physical point is withheld because of what the hand or the foot would be doing to a bone, a belly, a chest or a joint — and so the bars in this half are shorter, harder and more local: not if there is a fracture between L1 and S1; not if there is inflammation in the abdomen; not to heart patients, because pressure falls upon the chest; not to those who pass too much urine.

Because its effect can fall on the brain's CSF as well, therefore usually not more than (4) Down arrow are given.

That is one of only a handful of places in the atlas where an upper limit is put on a count, and the reason is printed beside it. Two things follow for a working therapist.

First, obey the limit. A card reading (6) Down arrow is a card to query, not to give.

Second, learn the shape. This is what the treatment reaches; this is why more of it is not better. When you come to write your own notes on a patient — and you should — that is the shape to write them in.

The low back, the sacrum and the tail bone

The largest physical family, and the one where the prohibitions bite hardest, because the hand here is over bone that can break and nerves that matter. Nothing in this family is given over the knee joint itself; The Low Back, "The Low Back", carries that rule with the points it governs, and it governs all of these.

J groin SupineWith this treatment we pull upward the tissue on the inner side of the knee that has descended, and bring it back to its own place. After all the other knee treatments, if a little pain remains on the inner side of the kaṭorī, this treatment is given to remove it—A finishing point, not an opening one. Given supine only.

BOF — Bottom of FeetAbove the heel, at the back, there is something like a band, through which pass the blood vessels of the feet and toes, the nerves that control movement, and the tendons — the ropes that keep the bones in their place — all of which come down from the knee or the waist and reach the feet. On striking something, or when we bend to lift a heavy weight, or make some sudden movement, many tendons and nerves and blood vessels are pulled and slip down quickly; afterwards not all of them are able to go back, and some get caught inside that band, from which there is very severe pain in the heel, or around the toe concerned. With this treatment we press those tendons caught inside the heel from below upwards and pull them, bringing them to their correct place, from which the patient gets immediate relief. Excellent for numbness, pain, burning or any kind of trouble from the toes, soles and heel up to the calves(see the note on sides)Ordinarily it is not given on the upper side. It is given only on the three sides of the calves. But if there is a sprain, then it is to be given on the upper side too — for which we write on the card BOF – all four sides. Blood supply to legs — for polioGiven to increase blood circulation in the arteries of the legs of polio patients. Give where there is numbness in the legs, or in the front or the back of the thighs. Blood supply to sidesFor those who have numbness in the side portion of the thighs, this treatment increases the flow of blood in that part and ends the numbness—Its own entry, and its own region — the side of the thigh, not the front or back. Method:
Hydrocele pointGiven for hydrocele — that is, swelling in the scrotum — and in inguinal hernia. Pressing at the appropriate part of the calf, where there is intense pain, increases the flow of blood in the testis of that side and the patient gets relief—A scrotal complaint found and treated in the calf.
VasantiThis is given to increase the circulation of blood in the veins. In diabetes, or in tarsal tunnel syndrome, when the feet go numb, give it from the heel towards the thigh. In carpal tunnel syndrome, when there is very severe pain, burning and numbness in the hands, give the treatment from the fingers towards the shoulder—The direction is the whole instruction, and it differs between the two conditions: heel → thigh for the foot, fingers → shoulder for the hand.

But now several other good treatments have come for fits — therefore it is not given—A treatment retired from one of its uses, with the reason given: better ones came.
Stretch — (6) ← → SupinePressing on the tips of the fingers, they are pulled outward, because of which it relaxes the muscles of the neck. It gives tremendous relief in breathing troubles. Therefore it is used in asthma, in bronchitis, and in the Oxygen formula—It is not necessary to give both sides at the same time. If the left side is given first and the right side afterwards, the result has been found just as good.
Shivaji trt. SeatedGiven to the patient who cannot raise the arm properly because of pain in the shoulder — the shoulder has become stiff, which is called frozen shoulder. With this treatment, by pressing again and again, we relax the muscles all around the shoulder, from which the blood circulation in them increases and flexibility comes, and the patient begins to get relief—

Three of those entries deserve a second look, because each teaches something about how this therapy handles its own history.

Electrical waves was withdrawn from one of its uses — formerly it was given in fits; but now several other good treatments have come for fits, therefore it is not given. Not because it failed. Because something better arrived. That is a therapy keeping a ledger.

Stretch says the two sides need not be simultaneous — if the left side is given first and the right afterwards, the result has been found just as good. A sentence written by somebody who tested it, for the benefit of a therapist working alone.

Tennis elbow and T8 set their counts by the patient — give until the pain goes out. Where the atlas can name a number it names one; where it cannot, it says so rather than inventing one.

The head and the face

Teeth point is the only entry in the physical half with a prerequisite. Before this, the appropriate heparin treatment according to the patient's body is to be given. Not a heparin treatment — the appropriate one, according to the patient's body, which is a whole family of formulas and a choice to be made. A card that reads Teeth point alone is a card missing a line.

Sulta Ulta in angina — the angle and the order are the treatment

This is one of the places where a student's instinct says surely not in a heart patient and the entry answers the instinct with detail rather than with reassurance.

It is given in angina. And it is given like this: both arms extended — not parallel to the shoulders, but a little upward, at 135°. And: not both sides at the same time. The left side first, and the right afterwards.

The angle and the order are not refinements laid on top of the treatment. They are the treatment. Given at 135° and one side at a time, it is the entry above. Given flat, and both sides together, it is something else that has not been taught.

And beside it, in the same patient, the other cardiac instruction of this article: in angina — that is, in heart patients — to reduce cholesterol, (2) Thrd is given in place of (4) Thrd. Each rule is complete where it stands.

The navel, the womb and the paralysed limb

Some of the bars in this article do not say never. They say not today, or not with that. They are the hardest kind to remember, because the point is not forbidden to the patient — it is forbidden in that company, and next week it may be exactly what is wanted. (Figure 25.8)

One disease, several points, one plan

Four polio entries doing four different jobs in one disease

Students meet polio in four separate entries of this article and are sometimes puzzled by it. They should not be. Read the four together and what appears is not a disagreement but a treatment plan for one disease, written across four points that do four different jobs.

(6) Adr is given for polio — because polio is an inflammatory disease, and (6) Adr is the point that ends inflammation, given for any disease whose name ends in -itis. Its own caution stands with it: it reduces immunity, so it is not given where immunity is already low — in septicaemia, in HIV, in cancer.

Blood supply to legs is given for polio — for the leg weakness, the numbness in the legs and the front or the back of the thighs, usually thirty times on both legs. That is the limb, not the inflammation.

The Armpit point and the Thymus and Thymus + Chest group are withheld in polio — they carry their own bar, printed with them, alongside diabetes and the autoimmune disorders.

And Spl carries an open question: to be tried on polio patients, to see whether they benefit from it? — printed as a question, and to be read as one.

Four points. One inflammation to end, one weak limb to supply, one group held back, one question still open. That is a plan, and it is how the whole of this article is meant to be read: not point against point, but each point doing its own work in its own condition, with its own qualifying detail attached.

Questions students ask

1. I am nineteen and in my first year. Do I have to learn all of this by heart? No — you have to learn where to look, and then look, every time, until your hand knows it. What you must hold in memory cold is the short list that changes a session: the points withheld in pregnancy, in menstruation, in diabetes, in cancer, in high and low blood pressure, in fits and from a growing child; the four paired instructions; and the six-second rule at (2) Pan. Everything else is a table, and a table is meant to be opened in front of the patient. A therapist who checks in the patient's presence looks careful. A therapist who guesses looks confident and is neither.

2. I am a housewife learning this to treat my mother. She has osteoporosis. Is half the article closed to her? Less than you fear, and the closures are precise rather than sweeping. Shukla is not given — for osteoporosis patients this treatment may perhaps increase the pain, so do not give them this treatment. (4) Para is not given alone; the full 1,25 DCC formula is given instead, and that is the important distinction. (4) Thrd is not given where a bone has grown at an unwanted place — spur, bamboo spine — and the bone indication for (4) Thrd is written for after the wound has filled in a break that is healing. 3. I am a physician. Some of the mechanisms here I would not write in a chart. What exactly am I being asked to accept? Two separate things, and they deserve separate pockets. The rules — this count, this side, this angle, this order, this ceiling, this bar — are a protocol, and a protocol is either followed or not. The explanations attached to them are the account the therapy gives of why the rule exists, and several of them are good physiology stated correctly: oestrogen closing the epiphyseal plate, parathyroid hormone mobilising bone calcium, alkalosis lowering the seizure threshold, the split between the two catecholamines down to the diastolic pressure. You may use the rules and hold the explanations lightly. What you may not do is discard a rule because you dislike its explanation.

4. I work in a village and I have no laboratory. Half these bars name a diagnosis I cannot make. Most of them name something you can ask about or see. Does she have periods now, and is she having one today? Is she pregnant, or might she be? Is there sugar? Is there pressure? Does he wheeze in the cold? How much urine is coming? Is there loose motion or constipation today? Has any bone been broken? That is eight questions and it closes most of this article's doors correctly. For the rest — a lump, a fever with rigors, blood where blood should not be — your skill is not the test; it is knowing where to send. 5. Why does Gal help skin disease in one row and harm an asthmatic in the next? Which is true? Both, and they are not about the same person. The middle column tells you what the point does and whom it helps; the right column tells you in whom that same doing is harmful. Gal is excellent in skin disease with constipation, where the benefit is great — and in asthma this point increases the trouble, so it is not given. A patient with psoriasis gets it. A patient with psoriasis and asthma does not. The point did not change; the patient did.

6. I am sixty-six and came to this late. The counts confuse me — (4) Ku is eight, and (½) Ku – 20 secs is one. How do I keep it straight? By reading the word after the number before you read the number. If the card says secs, it is a duration and the point is given once, held for that many seconds. If it says nothing, it is applications. And (4) Ku is the one exception announced in its own entry: two count as one — give it eight times, for six seconds each. Say the whole line aloud from the card before you touch the patient. Everybody does this; the ones who stop doing it are the ones who make the mistake.

7. A card came to me reading only P. What do I do? Send it back and ask which was meant. Three entries in this atlas hang a capital P off a shared stem and none of them is the others: P point, the prolapse point, which must be followed immediately by (6) WD; Thrd 'P', a point of the anterior pituitary with its own bar in bamboo spine and spur; and Ton 'P', the parotid version of Ton, not given to women who want children. 8. If (6) Adr is given for polio and the Armpit point is withheld in polio, how am I supposed to treat a polio patient? By giving each point the job it is written for. (6) Adr addresses the inflammation, polio being an inflammatory disease — with its own caution travelling beside it, that it reduces immunity and is therefore not given where immunity is already low. Blood supply to legs addresses the weak leg, thirty times on both legs. The Armpit point and the Thymus group are held back, carrying their own bar. And Spl carries an open question. That is four points doing four different things in one disease. It is a treatment plan.

9. My teacher says a short card is a good card. My patient thinks I am doing less for her because she is poor. Then say what is happening out loud, because she is right to notice and wrong about the reason. "Three of the treatments that would help your skin cannot be given to you because of your chest. I am not giving you less; I am giving you what is yours." A patient who has been told which doors are closed, and why, stops counting the points and starts watching the result. 10. Several entries have question marks printed in them. Are those mistakes? No, and they must never be tidied away. They mark claims their own author would not defend — four of the uses under (10) Pan, the athlete claim under (4) Pan, the polio line under Spl, the long Ku holds where research is continuing, and Dorsal. A student is entitled to know which sentences in a system are settled and which are still being explored, and a therapist who promises a patient something printed with a question mark has overclaimed.

11. How do I keep the same-session exclusions in my head during a busy morning? Do not keep them in your head. Keep them on the card. Write the day's plan out in full before the patient lies down and read it once, top to bottom, looking for pairs — Pradip with JJ⁺¹, BOF with JJ⁺², S4–S5 with Triangle. Three pairs, and they are the only ones. The mistake is never made by a therapist reading a written card; it is made by a therapist adding one more point halfway through because he has just thought of it.

12. What should I be writing down for the next generation? Exactly what this article contains and nothing grander. When a point helped, in whom, at what count. When a point did not. And above all, when a point made somebody worse — because the rarest and most valuable rules here are the ones nobody deduced. In asthma this point increases the trouble. This is a matter of experience. Somebody paid for those lines, once, and wrote them down so that nobody would have to pay again. Questions patients ask

1. "You gave my neighbour six treatments and me only two. Is it because of the fee?" No — it is because of your chest. Four of the treatments your neighbour gets are the ones that make a wheeze worse, and one of them is the one I would most have liked to give you for your skin. I am not holding anything back to save time. I am leaving out the things that would cost you a bad night, and what remains is chosen for you rather than for your diagnosis.

2. "My sister was given a treatment for her knee and you are not giving it to me. Same knee, same age." Same knee, different back. There is a treatment for the middle of the back that is not given to somebody with knee pain, because it pulls the muscles the opposite way from the knee treatment; your sister has no knee pain and you have. She gets the one; you get the other. In a month, when the knee is settled, the question will look different and I will ask it again.

3. "Is this safe in pregnancy?" Parts of it are and parts of it are not, and I will tell you which as we go. Two treatments here are not given to pregnant women at all — after them the babies move about vigorously, and it is judged that the baby is uncomfortable. There are things I would ordinarily use for nausea that I will give with the hands rather than the feet. If you are pregnant, or think you might be, tell me before you lie down and not afterwards. It changes the plan and it is not a difficult conversation.

4. "I have my period. Should I come?" For other problems, no — come next week. But do come during your period for these: bleeding that is far too heavy, bleeding that will not stop, bleeding that is very scanty, or pain and clots. Those are treated during the period, not despite it. And one of the treatments I would usually give you is not given to a woman during her period, which is the other half of why I ask.

5. "I heard this treatment stops a woman conceiving. Is that true?" Here is exactly what is known, and I would rather you had it from me than from the lane. There is one treatment that may perhaps affect conception, and for that reason it is not given to any woman who wants a child. But it is not proven to prevent conception, and nobody may call it a contraceptive treatment. So do not use it as one, and do not let anybody tell you it is safe to rely on. If you want contraception, let us talk about where you can get something that has actually been tested.

6. "Why do you keep asking about my urine? I came about my back." Because two of the treatments I might give you for that back work on the same set of muscles that control your water, and they pull opposite ways. One of them opens and is not given to somebody already passing too much urine; the other tightens and is not given to somebody passing too little. So the answer to how much urine is coming decides which of the two you get. It is a short question with a long consequence.

7. "You said this point ends lumps. My mother has a lump. Will you give it to her?" I will not give anybody a treatment for a lump that nobody has examined. Bring the report first — not because I doubt you, but because the treatment for a lump that has been identified and the treatment for a lump that has not are not the same act. And I will say this plainly: if it turns out to be something serious, this therapy goes alongside whatever her doctor gives, never instead of it. Anybody who tells you otherwise is selling you delay.

8. "My blood report is normal. So why am I being treated at all?" A normal report is good news and worth having — the things that test looks for have been ruled out. It does not mean nothing is wrong with you; it means nothing that this test measures is wrong with you. Much of what troubles people is an organ working sluggishly rather than an organ being damaged, and no blood report is built to show that. What I can tell you about is what I find with my hands, and I will tell you honestly whether it changes.
