Where a blood pressure falls from, and what falling does

Haemorrhage, hypovolaemia — and the signs that say the system did not answer

Now let us see from what causes BP can fall. If low BP has come on suddenly, one main cause of it is blood suddenly flowing out of the body. This is called haemorrhage. Because of it, the blood volume — the quantity — falls all at once.

Ordinarily this can happen from the following causes:

when some deep injury has occurred;

or an accident has happened;

or, in women, when there has been very heavy bleeding during an abortion — a miscarriage — or in the menses;

some people can feel giddy even after donating blood. That too is from this same cause.

Note: if bleeding has occurred because of an accident, in the hospital report it is written as haemorrhage due to RTA — RTA meaning Road Traffic Accident.

What trouble is caused by BP falling

If the bleeding is suddenly so great that 35% of the body's blood — around one and three-quarter litres — goes out, then because of it, first the outflow of blood from the heart will be very much reduced, and afterwards the BP will keep falling. Then heart failure can also occur — that is, the heart may even stop altogether — which is called hypovolaemic shock. So that this should not happen, angiotensin #2 constricts the peripheral arteries more, so that the situation of heart failure should not arise.

[hypo = less; vol = volume; -aemia = in the blood. Hypovolaemia means the volume of blood becoming low.]

If around half a litre of blood goes out of the body, there is not so much danger. In some women this can happen after the menses — because of which, for two to three days after the menstrual period, or for somewhat longer, they feel giddy on doing any work at all. On being asked they will say that their bleeding in the menstrual cycle is very heavy. Because of this their blood volume and blood pressure fell suddenly for some days.

And then the sentence that turns a physiological account into a way of examining a woman standing in front of you.

A shortage of blood means a shortage of haemoglobin — the oxygen-carrier — in the body. When the woman is lying down, the blood she has is enough to supply the oxygen needed for the body's involuntary work — the work of the heart, the lungs and so on. But when she sets to work, she needs more oxygen, which she cannot get because of the shortage of haemoglobin. Because sufficient oxygen does not reach the brain, she feels giddy.

The other signs in such women are shivering, clenching of the teeth, speech stopping — that is, not even having the strength to speak — and falling temperature and pulse.

So if, after blood has suddenly been lost, the above signs appear in someone, one meaning of it is that angiotensin #2 is not being formed in their body.

(The production of angiotensin number two increases when the pressure in the arteries, or the quantity of blood in them, suddenly falls. So where the pressure has fallen and the signs of the fall are there, the system that should have answered it has not answered.)
