The Neurotherapy (LMNT) diagnosis card: what is written and why

Every Neurotherapy (LMNT) patient has a diagnosis card. It records the complaint in the patient’s own words, a baseline, the graded pain points, the acid or alkali direction, the reports, the blocked points and the first work, so that any therapist can follow and check the course.

Overview

The blocked points are written before the plan is built, and nothing above that line is ever erased.

The card opens with who the patient is, how far they travel and which days they can honestly come. Then comes the complaint in the patient’s own words, and the complaint mapped: since when, where, what makes it worse, and what they can no longer do.

Next is the baseline: hunger, sleep, energy, weakness, lightness and glasses of plain water drunk yesterday. Digestion and bowel come after, with tongue, stool form, effort and any undigested food, followed by a head-to-foot checklist, daily life, past illness and everything swallowed, including home remedies.

The findings block lists each pain point with its side and grade, including the points that do not hurt. From this comes the direction, acidic or alkaline, and a diagnosis written as organs, direction and duration, never as a disease name. Each report is one dated line, marked changed the plan or changed nothing.

Before any plan, the blocked points for the patient’s conditions are written in their own box. Unblocking is a dated, named decision. Then the first work, usually digestion, the two rules given today, what will be measured, and anything referred to a doctor. A running record follows, one line per session.
