One line per report. Never a paragraph.

Column · What goes in it

Date of the report · The date the sample or picture was taken — not the date you saw it

Test, value with its unit, and the laboratory's printed range · Copied exactly, in its own words — the next laboratory may print another range

Changed the plan / changed nothing · In those words. No third option — with the repeat date, if a doctor asked for one

What changed · Mu° and Liv° withheld · sent to Dr ——, 14th · or a dash

Sugar and BP are marked in their proper place on the card, and the chief therapist is informed. Osteoporosis is written where it cannot be missed — hand only ▲ Take care . A joint-disease report is written as a date and a result and nothing more.
